2. Date of your ban I don't know, long time ago to even remember
3. What game/server are you banned from all I think
4. Who banned you (only if you know who) no idea, forgot
5. Reason why you got banned (only if you know why) cheating, probably sethhack ( i had it a long time ago )

6. Reason why we should unban you it was a few years ago when I used sethhack, I actually got banned from the cheat, because after a month I got it, I called it a shit hack

I believe your case was already discussed in this topic: forums.mrgreengaming.com/topic/13757-new-player-banned-before-joining/ with a 'No Unban'
